15 Minute Zoom/phone consultation to discuss your pet - $50.00

Please be aware that what can be accomplished during these appointments without a physical exam is limited and up to the discretion of Dr. Polk. $50 per 15 minutes

If it is determined that an in-home evaluation is needed, the cost of the telehealth consultation will be subtracted from the comprehensive exam.

(Discount limited to a single 15 minute telehealth fee of $50.00.)

Full evaluation of vitals, body systems, and general health. Discussion of preventative health, disease management, and quality of life plans. Creation of diagnostic and therapeutic options. Referrals may be needed depending on discussion/findings.

• New PATIENT (not client) - $250

• Existing Patient - $150

Consultation of medications, dosage adjustments, and prescription management. 15 min zoom call is required.

• Feline Viral Rhinotracheitis-Calici-Panleukopenia Vaccine - $55.00

Euthanasia + Private Cremation - $600.00

Euthanasia + Communal Cremation - $450.00

Euthanasia with at Home Burial - $350.00

Based on miles driven and duration of commute. An accurate estimate can be provided once an address is entered into our system. Travel fees are incurred with every home visit.
